Date:

Encourage accountability and reinforce positive student behavior with this Behavior Contract Template. Designed for all grade levels (PK-12), this tool helps educators and students collaboratively set clear expectations, define replacement behaviors, and establish reinforcement strategies. With a built-in progress monitoring system, teachers can track improvements and adjust goals as needed.

How to Use the Behavior Contract Template:

  1. Identify the Target Behavior – Work with the student to pinpoint a behavior that needs improvement.
  2. Define the Replacement Behavior – Clearly outline what the student should do instead.
  3. Set Measurable Goals – Determine how often the replacement behavior should occur.
  4. Establish Reinforcement – Choose meaningful daily and weekly rewards for meeting behavior goals.
  5. Monitor and Review – Use a tracking system and schedule regular check-ins to assess progress.

This resource is part of Teaching Channel’s Replacement Behaviors Intervention Guide, which offers a complete eBook of strategies and downloadable tools for classroom management, tailored to every grade level.
